Immigration officials did not document the medical necessity of at least two hysterectomies they authorized for women in their custody, according to a new report<\/a> by the Department of Homeland Security\u2019s inspector general.<\/p>\n Investigators contracted with an OB\/GYN to review six hysterectomies performed on migrant women who were in federal custody.
